Left atrial appendage thrombus; young or old? Role of CMR in definition

چکیده
Methods An 83-year-old male with history of AF, DM, cardiomyopathy presented with acute weakness and numbness of the left upper extremity. Two weeks prior to his presentation his INR reportedly ranged from 1.1 to 1.6. However, his INR level was 2.3 at the time of presentation. Upon examination, he was alert and vital signs were stable. Cardiovascular examination revealed irregularly irregular rhythm without any murmurs. Neurological evaluation revealed normal cranial nerves, mild weakness (4/5) of the left upper limb, normal motor strength over the rest of the limbs, normal sensory examination, gait and reflexes were normal.

Juxtaposition of the atrial appendages: A nidus for thrombus in atriopulmonary Fontan?
Juxtaposition of atrial appendages is a rare cardiac congenital anomaly, usually associated with other cardiac malformations. Until now, it has not been linked to any significant clinical implications. We report cardiovascular magnetic resonance (CMR) findings of two adult patients who underwent atriopulmonary Fontan operation in the setting of left juxtaposition of the atrial appendages. The p...
متن کاملIs cardiovascular MRI equally effective as TEE in evaluation of left atrial appendage thrombus in patients with atrial fibrillation undergoing pulmonary vein isolation
Introduction Patients with atrial fibrillation (Afib) routinely undergo a transesophageal echocardiogram (TEE) for evaluation of the left atrial appendage (LAA) to rule out thrombus prior to undergoing cardioversion or pulmonary vein isolation (PVI). Cardiac MRI (CMR) is now increasingly used for evaluation of these patients for defining pulmonary vein anatomy prior to PVI. We hypothesized that...
متن کاملLeft atrial appendage mass: is it always a thrombus?
Myxoma is the most common benign tumor of the heart, but it is very rare for it to originate from the left atrial appendage. Distinguishing between a mass, a thrombus, and a tumor in the body of the left atrium with preoperative transthoracic or transesophageal echocardiography is very difficult, even more so in patients with mitral valve disease and chronic atrial fibrillation. A 50-year-old m...

متن کاملA Semi-Automated Algorithm for Segmentation of the Left Atrial Appendage Landing Zone: Application in Left Atrial Appendage Occlusion Procedures
Background: Mechanical occlusion of the Left atrial appendage (LAA) using a purpose-built device has emerged as an effective prophylactic treatment in patients with atrial fibrillation at risk of stroke and a contraindication for anticoagulation. A crucial step in procedural planning is the choice of the device size. This is currently based on the manual analysis of the “Device Landing Zone” fr...
